Naturopathic Forms are PDF fillable, you can fill them in and email them to us to print or simply print them off and fill them out to bring into the office on the day of your appointment. All other intake forms can be printed off at home and brought in on the day of your appointment or filled out in the office.

Frequently Asked Questions

What to Expect as a New Patient?

  • Please have your Naturopathic Intake Forms filled out prior to your appointment. You can either email them into the office to print off or bring them with you. All other intake forms can be filled out in the office.

  • Please arrive 15 minutes prior to your appointment. Upon arrival at the clinic, check in with the front desk, hand in your intake forms and please have a seat in our waiting room. There is a washroom available for patient use if needed.

  • Please see our location and office hours here.

  • You can access our free parking lot off of either Hibernia Street or Huron Street.

  • The frequency of follow up visits will be dependent on your health goals and your initial appointment with your practitioner.

  • Every insurance plan is different. It is important for you to find out what specific coverage you have prior to your appointment. We do not do direct billing to insurance companies, so you will pay upfront and then submit your receipts.

  • We accept cash, debit and credit and payment is due at the end of your appointment.

  • If you need to cancel and reschedule your appointment, we ask that you give us 48 hours notice. If you cancel within 48 hours of your appointment you will be charged 50% of your appointment fee.
